Your chronological age is just a number. Your biological age is what your body’s internal systems actually look like under the hood. Researchers measure it using blood chemistry markers — things like albumin, creatinine, and inflammation signals. Two widely used tools, called KDM-BA and PhenoAge, take those markers and calculate how fast your body is truly aging. The gap between those two numbers turns out to matter a great deal for your mental health.
A 2023 study published in Nature Communications tracked more than 424,000 midlife and older adults in the United Kingdom Biobank over a median of 8.7 years. Adults with accelerated biological aging were more likely to already have depression or anxiety at the start of the study. They were also more likely to develop those conditions during the follow-up period. Both the KDM-BA and PhenoAge tools pointed to the same conclusion, which makes the finding harder to dismiss as a fluke.[1]
The Finding That Makes This Study Hard to Ignore
What gives this study real weight is what it ruled out. The researchers found the same elevated risk even after controlling for genetic risk scores and self-reported childhood adversity.[1] That matters because critics often argue that mental health outcomes are really just tracking inherited vulnerability or early-life trauma. This study says no — biological aging adds something on top of those factors. Northwestern University’s Feinberg School of Medicine summarized the findings by noting the link held regardless of a person’s genetic risk or medical history.[2]
The effect sizes are real but not enormous. The increased risk per standard deviation of biological-age acceleration lands in the single to low double digits, percentage-wise.[1] That makes biological aging a contributing factor, not the whole story. Think of it as one dial among several — but a dial that researchers now have reason to watch closely.
The Feedback Loop Nobody Talks About
Here is where it gets more complicated — and more important. The relationship between aging and mood does not run in just one direction. Research published in Frontiers in Aging suggests that depressed mood itself can accelerate biological aging.[4] Chronic psychological stress is also linked to faster cellular aging through mechanisms involving inflammation, oxidative stress, and DNA damage.[17] Feeling unhappy or lonely has been shown to add roughly 1.65 years to a person’s biological age in some studies — an effect that outweighs smoking status or where you live.[18]
So the picture that emerges is a feedback loop. Faster biological aging raises your mood disorder risk. Depression and anxiety, if they take hold, can then push biological aging faster. Breaking into that loop early — before it gains momentum — is exactly why researchers think biological-age screening could eventually serve as a prevention tool, not just a diagnostic curiosity.[1]
What the Study Cannot Tell You Yet
The honest read on this research is that it shows association, not confirmed causation. The study is observational — nobody was randomly assigned to age faster. Lifestyle factors, socioeconomic stress, medication use, and hormonal changes could still be doing some of the work here.[1] The study also focused on midlife and older adults, so the findings are less certain for younger people. And the combined depression-and-anxiety outcome used in parts of the analysis means the evidence is less sharp for each condition on its own.[1]
What science still needs are intervention trials — studies that actively slow biological aging through exercise, anti-inflammatory strategies, or metabolic programs, then track whether depression and anxiety rates actually fall. Until those trials exist, the causal arrow remains an educated hypothesis, not a settled fact. But the hypothesis is now backed by half a million data points, and that is not nothing.
What You Can Do With This Information Right Now
You cannot change your birth year. You can change how fast your body ages. Diet quality is directly tied to biological-age acceleration — a more inflammatory diet raises the odds of faster aging by at least 25 percent in some measures.[22] Chronic stress, loneliness, poor sleep, and sedentary behavior all push the biological clock faster.[17] Exercise, social connection, and managing inflammation work in the opposite direction. The science does not yet guarantee that slowing your biological age will protect your mood. But the evidence strongly suggests the two are linked — and the levers for slowing biological aging are the same ones your doctor has been recommending for decades.
